Covered stents seem to be an effective tool to seal severe perforations that have persisted despite prolonged balloon inflation. However, the high profile and low flexibility of previous covered stents compromise the deliverability of these devices, particularly in emergency situations. The Mguard stent (InspireMD, Tel Aviv, Israel) is a novel mesh-covered stent that has been designed to prevent distal embolization in thrombotic lesions. We herein report two cases that describe a new potential use of the Mguard stent for sealing perforations in coronary circulation.
